Summary
The Cincinnati VA Medical Center is seeking a Nurse Practitioner to float between our Dearborn and Hamilton Clinics. The NP will provide a full range of services emphasizing quality - timeliness - and advancement of the VA mission by providing advanced clinical practice and consultation to the full extent of their education - training - and certification. - They will practice in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
- The NP provides advanced clinical practice - consultation - and management
- The NP practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position
- The NP has prescriptive authority for a panel of patients or program as indicated by their position
- The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Major duties may include but not limited too: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

- Supervisory Controls: Refer to the facility organizational chart for the supervisory chain-of-command and physical location of this position
- The incumbent is responsible for self-directed performance of the approved clinical privileges

- Relocation/Recruitment Incentives: Not Authorized EDRP Authorized: This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program
- You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment
- Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application
