Job Summary
The Pharmacist will be responsible for delivery of quality and efficient pharmacy services in line with the established protocols as well as the Hospital’s patient care objectives and the licensing board guidelines and policies. The position will be monitoring the supply of all medicines used in the hospital and will be in-charge of dispensing prescribed drugs, performing quality checks on medication stock and raising a requisition for the required drugs to replenish the stocks.
Duties and responsibilities:
· Receive a new prescription order from physician/doctors.
· Perform identification, evaluation, interpretation, and clarification of a prescription.
· Verify the prescription before you release to the patient.
· Consult with a patient regarding a prescription or non-prescription drug.
· Perform a professional consultation with any physician/doctor.
· Dispense pharmacy supplies and keep accurate reports on drugs and other pharmacy supplies
· Offer advice to patient on how to take a medicine
· Ensure compliance with professional and legal practice and standard operating procedures
· Undertaking clinical checks on all treatment sheets and prescriptions for suitability of treatment.
· Monitor drug therapy and carry out clinical pharmacy interventions for both in patients and outpatients in liaison with the patient care team.
· Reporting, Monitoring and analysis of medication errors, adverse drug reactions and other quality and patient safety indicators.
· Carry out medication use counselling and adherence monitoring for patients on chronic treatment.
· Ensure institutional compliance with antibiotic stewardship guidelines restrictions
· Undertake medication reconciliation during patient admission and discharge.
· Develop guidelines and standard operating procedures.
· Prepare and submit all monthly, quarterly, and annual reports on consumption of medicines, non-pharmaceuticals, and overall stock status in line with requirements.
· Provide information and education on drugs and medication therapy to patients, healthcare workers and other hospital personnel.
Minimum Qualification:
· Diploma in Pharmacy from a recognized Institution.
· Bachelor’s Degree in Pharmacy added advantage.
· Valid membership and hold a Practicing license with the Pharmacy and Poisons Board of Kenya which must be available for use by the hospital if needed.
· A minimum of two (2) years clinical pharmaceutical experience.
· Experience in clinical research is advantageous.
· Proficiency in use of health information systems
