Job Description/Requirements
- Process incoming healthcare service referrals by reviewing patient needs and aligning them with appropriate providers based on service type, location, availability, and capacity.
- Facilitate communication between referring organizations, service providers, and internal teams to confirm care arrangements, provide updates, and ensure clarity throughout the coordination process.
- Track and manage referral statuses from intake to completion, maintaining oversight of deadlines, progress updates, and any changes in care delivery plans.
- Maintain accurate and confidential records of all healthcare referrals, communications, and service outcomes in accordance with regulatory and organizational standards.
- Utilize electronic medical record (EMR) systems and other healthcare software platforms for data entry, case tracking, and provider information management.
- Assist with verifying provider qualifications, service capabilities, and availability to maintain a responsive and compliant provider network.
- Execute assigned tasks using detailed checklists and structured workflows to ensure consistency, quality, and regulatory compliance in all service coordination activities.
- Use Microsoft Office Suite (Word, Excel, Outlook) to generate documentation, maintain trackers, and communicate with stakeholders.
- Apply high attention to detail to all aspects of coordination work, particularly in the management of patient information, service documentation, and data accuracy.
- Maintain flexibility to shift priorities as needed while supporting multiple ongoing patient care cases in a dynamic healthcare environment.
- Demonstrate professional, empathetic, and clear communication in all patient- and provider-facing interactions.
